How To Contact Your Mayor for a CDH Awareness Day Proclamation

It is very simple to request a proclamation from your town’s mayor.   Simply go to their web site and find their address or a contact form.   You can copy the text below to use.  Edit the text in BOLD and add text in the blanks to your own info / town name and send.   If you receipt a proclamation, please scan it and e-mail to us at awareness@cdhi.org so that we can share.

Our charity, CDH International, would like to officially request a proclamation for “Congenital Diaphragmatic Hernia Awareness Day” for April 19, 2018.

All 50 states and the United States Senate recognize this every year (we are still working on the House).   My family would really like to include our hometown this year please.

Verbiage is below:

WHEREAS:One in every 2,500 pregnancies are diagnosed with a congenital diaphragmatic hernia (CDH); and

WHEREAS:Since 2000, it is estimated that over 500,000 babies have been born with CDH; however, only 50 percent of those babies survived; and

WHEREAS:CDH is as common as spina bifida and cystic fibrosis; however, very few people know about it or are aware of it; and

WHEREAS:1,600 babies are born with CDH every year in the United States each year; and

WHEREAS: There are many people living in North Carolina who have been diagnosed with and have survived their CDH, although many families in North Carolina have endured the horrible pain and grief associated with the loss of loved ones with CDH; and

WHEREAS: There are over 900 families affected by CDH living in the state of North Carolina who are members of the charity, CDH International; and

WHEREAS: CCDH International is a charity is a 50(c)III organization that was created to help families affected by CDH; and

WHEREAS: Those with CDH often endure multiple surgeries and possible medical complication beyond their diagnosis that include heart defects, pulmonary complications, gastric and intestinal problems, developmental delays, and may require respiratory and medicinal support for years; and

WHEREAS: Raising awareness of this congenital defect will help bring about acceptance and support for those suffering with it and will help advocate for urgently needed medical research and advances.
